As healthcare data grows, many facilities find themselves in a financial squeeze. Storing petabytes of imaging and patient records in a public cloud creates a “data gravity” problem: it is easy to put data in, but expensive and slow to pull it back out for daily clinical use.
The solution isn’t to abandon the cloud, but to shift its role. Instead of using the cloud as a complete solution, we use it as a secure front gate.
The Architecture:#
Cloud as the “Frontend,” Facility; In this model, the heavy lifting happens on-site, but the user experience feels like a modern cloud app. Here is how the “Handshake” works:
- 1. The Local Secure Core (The Database)
The actual patient records, FHIR databases, and high-resolution images live on secure local servers within your facility.
Why?
high-speed internal access for staff, and total physical control over the encrypted drives.
- 2. The Cloud Frontend (The Interface)
You host a lightweight application or API gateway on a public cloud provider (AWS/GCP) or a high-performance VPS.
_Why? _
Public clouds have excellent global uptime and DDOS protection. They act as the “Gateway” of your system that doctors/staffs/patients can access from anywhere.
- 3. The Secure Handshake (The API)
When a doctor logs into the cloud frontend, the system performs a secure, real-time handshake with your facility’s internal API.
How?
Instead of opening your database to the internet, you use a Reverse Proxy or a Secure Tunnel (like a Cloudflare Tunnel or a WireGuard-based VPS). The cloud frontend “asks” for the data, and your local server “pushes” only what is needed for that specific request.
The Multi-Cloud & VPS Advantage#
By using this handshake method, you gain the freedom of a Multi-Cloud strategy:
Vendor Independence: If one cloud provider prices shift, you can move your lightweight frontend to a different VPS provider in minutes. Since your data is safely in your facility, there is no massive data migration required.
Reduced Attack Surface: Since your database is not “on” the internet, a breach of the cloud frontend doesn’t automatically mean a breach of the medical records. You can automate “sever the handshake” to protect the internal data.
Cost Control: You only pay for a smaller virtual machine (VPS) or Cloud fees to run the interface, rather than paying for terabytes of premium cloud storage.
The Lean Infrastructure Mindset#
Implementing this requires a paradigm shift. You move from being a “Cloud as completed solution” to a “Mixed Architect Solution.”
Encryption: Data is encrypted locally, and the “Handshake” is protected by modern TLS protocols.
Access Management (IAM): You use the cloud to manage who can log in, but your local server manages what they can see, access, excute, or level of permisison.
Quick Comparison: Cloud-Only vs. Cloud-Fronted#
| Feature | Cloud-Only Solution | Cloud-Fronted Facility |
|---|---|---|
| Data Gravity | High (Hard to move) | Low (Data stays local) |
| Monthly Cost | High (Scales with data size) | Low (Fixed hardware + small VPS) |
| Security | Internet-exposed Database | Air-gapped / Tunneled Database |
| Performance | Network-dependent | LAN speeds for on-site staff |
